Endoscopic ultrasound-guided pancreatic duct drainage: technical approaches to a challenging procedure.

چکیده
Endoscopic ultrasound-guided pancreatic duct drainage (EUS-PDD) is an effective treatment modality for pancreatic pathologies, including chronic pancreatitis, major or minor papilla inaccessible via endoscopic retrograde cholangiopancreatography, and postsurgical pancreaticoenterostomy stricture [1,2]. Technically, however, EUS-PDD can be very difficult and is potentially associated with significant complications [3]. The following series of videos aims to improve the understanding and performance of EUS-PDD by describing the three approaches to this challenging procedure (●" Video1): the transluminal stenting, anterograde drainage, and rendezvous method. To our knowledge, this is the first published video series focused on the different methods and steps of this intricate procedure. Case 1 describes a 55-year-old manwith a pancreaticoenterostomy stricture following Whipple surgery. It showcases the transluminal approach, where a stent is placedupstreamfromthestrictureforming a pancreaticogastrostomy (●" Fig.1a– c, ●" Video 2). The patient’s symptoms improved dramatically following the procedure. Case 2 is another example of the transluminal approach; however, this time it demonstrates the creation of a pancreaticoduodenostomy with the insertion of

Endoscopic Ultrasound-Guided Pancreatic Duct Intervention
Endoscopic ultrasound-guided pancreatic duct intervention (EUS-PDI) is an emerging endoscopic approach allowing access and intervention to the pancreatic duct (PD) for patients with failed endoscopic retrograde pancreatography (ERP) or patients with surgically altered anatomy. As opposed to biliary drainage for which percutaneous drainage is an alternative following failed endoscopic retrograde...
متن کاملEndoscopic Ultrasound-Guided Pancreatobiliary Endoscopy in Surgically Altered Anatomy
Endoscopic retrograde cholangiopancreatography (ERCP) has become the mainstay of therapy for pancreatobiliary diseases. While ERCP is safe and highly effective in the general population, the procedure remains challenging or impossible in patients with surgically altered anatomy (SAA). Endoscopic ultrasound (EUS) allows transmural access to the bile or pancreatic duct (PD) prior to ductal draina...
متن کاملStudies of endoscopic ultrasound-guided drainage for pancreatic pseudocysts and abscesses: a university hospital-based experience
Background: Recently, endoscopic ultrasound (EUS)-guided drainage is used to treat pancreatic pseudocysts and abscesses. Endoscopic necrosectomy is now rarely performed clinically. We retrospectively studied the effectiveness and complications of these therapeutic procedures. Methods: The subjects comprised 26 patients with symptomatic pancreatic pseudocysts, pancreatic pseudocysts 5 cm or more...

It is very difficult to define today the place of EUS-guided pancreatic duct drainage; in our experience, the best indication is anastomotic The development of interventional EUS has provided better access to the region of the pancreas. Just as pancreatic fluid collections, such as pseudocysts, can be successfully drained from the stomach or duodenum by endoscopic cystenterostomy or cystgastros...
متن کاملEndoscopic ultrasound-guided methylene blue injection to achieve bile duct cannulation after failed ERCP.
Endoscopic ultrasound (EUS)-guided biliary drainage is an increasingly popular technique for cases of previously unsuccessful endoscopic retrograde cholangiopancreatography (ERCP). It has good results in expert hands, but is a challenging procedure with a significant rate of adverse events [1]. EUS-guided methylene blue cholangiopancreatography is a wireless alternative to the rendezvous techni...
